Helping Heal in Your Home
Our Senior Home Nutrition Program—also known as the Discharge Meal Program—provides older adults returning home after a hospitalization with eight weeks of balanced meals. Participants receive seven frozen meals each week, along with a wellness check from a Senior Resources team member.
This program is designed to ease the transition from hospital to home, reduce readmission rates, and connect seniors to long-term nutrition services when needed. By addressing key social determinants of health, it improves outcomes for high-risk seniors in Richland County.
